Mike,
You are correct in thinking that the 16mm outlet from the thermostat housing goes to the 16mm outlet on the cut and shut pipe. You should also connect the bottom outlet from the header tank into the bottom hose from the radiator. I didn't do this instead using the rad fan switch hole in the radiator to connect the header tank into it. You can see the adapter screwed into the rad in the picture.
